What’s new in the box of goodies this month?
‘A Systematic Approach to Chronic Disease Management and Care Planning’ Training CD is our latest edition to the Brainbox training packages. Launched this month in the General Practice Queensland and the Australia Practice Nurses Association newsletters, this training has something to offer any one involved the management of chronic diseases.
A little different to our pure software training packages, this training incorporates the use of software, templates and expert advice on ‘How To’ optimise your patient database to create accurate registers to identify patients at risk and suffering from a chronic illness. This is the first step in this unique systematic approach to CD Management and will set you on the path of successful completion of the GP Management Plans, Team Care Arrangements and the Domiciliary Medication Management Review. The training has been designed to enhance the way GP’s, Practice Nurses and Allied Health workers collaborate in your practice to achieve better health outcomes for your patients. The patient being the primary focus…with emphasis on the patient taking a proactive role during the goal setting phase of the care planning process.
Another feature of this CD Training is the expert tips and tricks on how to modify and import templates and adding reminders. Our Trainer Margaret Windsor RN, has some brilliant ideas on how to simplify the templates and make them more user friendly, especially useful when it comes time for recalling patients. And to complete the system, a quick look at how to navigate thru the new clinical audit tools, MDStats, PEN CAT and the DCP (Doctors Control Panel).
As an added bonus: This CD-ROM has been endorsed by the Royal College of Nursing Australia according to approved criteria. Completion attracts 2 RCNA Continuing Nurse Education (CNE) points as part of RCNA’s Life Long Learning Program (3LP).
It really is a fantastic all-in-one resource that will:
- save you time
- help to streamline your current assessment methods
- make it easy to claim the MBS item numbers
- give practice nurses the opportunity to earn points while they learn and
- encourages patient self-management.
